Nurse Practitioner, Hospice – Full-Time – Salaried

The Role: The Nurse Practitioner, under the direction of the Hospice Medical Director, will be available to function as attending physician for select hospice patients, consult on hospice, and selected palliative care patients, and support the Hospice Medical Director and Physicians. The nurse practitioner will share in the on-call rotation with the hospice physicians. The Nurse Practitioner interfaces with other managers, hospice physicians, hospice interdisciplinary team members, and community physicians to ensure excellent patient care and customer satisfaction.

· Emotional maturity to cope with the ongoing needs of hospice care.
· Ability to perform complex written and verbal communication in English, face-to-face and in group settings.
· Ability to frequently sit, stand, and walk.
· Ability to make repetitive motions involving hands/wrists involved in writing, keyboarding, and using the computer mouse.
· Ability to lift and carry up to 50 lbs. unassisted.
Requirements:
· Must have at least five years of nursing experience, part of which has been community based
· Must be an adult, family, or geriatric nurse practitioner
· Direct hospice or palliative care experience is helpful
· MSN from an accredited university or college
· Education and/or experience to meet nurse practitioner requirements as defined by the Oregon State Board
· Must possess a current Registered Nurse license in the State of Oregon and licensed as Nurse Practitioner by Oregon State Board of Nursing.
· Computer literacy including use of Electronic Medical Records (EMR) required
· Must have prescriptive authority
· Must be vaccinated for COVID 19
· Must have a valid Oregon Driver’s license, proof of insurance and reliable transportation
Responsibilities:
· Acts as expert consultant or attending physician for hospice patients as assigned
· Makes home visits to patients and families as needed
· Consults on pain/symptom management issues
· Evaluates patients to provide information for recertification discussions
· Shares in the hospice physician on-call rotation, with back-up available at all times from the hospice physicians
· Serves as a source of clinical expertise to hospice staff, agency, providers, and payer sources.
· Transfers information accurately throughout the organization and to the clinical staff.
· Acts as resource for prescriptions when attending physician unavailable
· Collaborates with Hospice Medical Directors and Physicians regarding clinical issues.
· Participates in interdisciplinary group discussions and meetings when appropriate or requested
· Reviews patient charts and medications as requested and provides input on appropriate interventions or changes in the plan of care
· May perform or assist in medical procedures at the patient’s residence, including intravenous or subcutaneous line placement, phlebotomy, and wound care
· Demonstrates continued professional growth and development through participating in education programs and review of current health literature.
· Maintains clinical knowledge to ensure the provision of excellent clinical care and to serve as resource to staff.
