Westchester Independent Living Center/Putnam Independent Living Services is a consumer-controlled, non-residential organization that provides information and advocacy services to persons with disabilities. In collaboration with ACCES-VR, provide an enhanced component of service delivery for consumers with disabilities that promotes self-reliance, recovery, self-advocacy, self-image, and self-determination/self-sufficiency. Provide emotional support and information and facilitate linkages to community resources.

JOB TITLE: Education & Outreach Training and Coaching Specialist

PROGRAM: Open Doors Transition Center (NYAIL)

JOB SUMMARY: Open Doors Transition Center Program assists identified individuals to transition from institutional settings to the community. This position covers the Lower Hudson Valley (LHV) including Ulster, Sullivan, Dutchess, Orange, Rockland, Putnam, and Westchester. The Education & Outreach Specialist will have 3 main functions.

• Education & Outreach presentations: Contacting nursing homes and doing presentations on Section Q and The Open Doors Program with a pre-set PowerPoint presentation.
• Good Neighbor outreach: Outreach to nursing homes for the Good Neighbor program (GNP) and assisting the Regional 10 Transition Specialists connect with the GNP.
• Post Transition Person Centered Plan (PCP) Coaching: Working with participants who are in their first 60 days post transition and implementing a pre-set PCP curriculum that will involve primarily face to face contact with participants in their homes. Must be capable of providing strong Service Coordination support for select transitions.

REPORTS TO: Open Doors Program Coordinator – Regional lead

QUALIFICATIONS:

1. Bachelors Degree and minimum of 2 years work experience providing Service Coordination for individuals with Long-term physical & mental disabilities.

2. Experience developing and providing trainings to groups and comfortable with public speaking.

3. Ability to travel throughout the 7 counties of the Lower Hudson Valley.

4. Must have excellent verbal and written communication skills

5. Must be well organized, able to work independently and able to meet deadlines.

6. Must be computer literate and able to learn and navigate a variety of programs.

7. Should have interests in disability rights and familiarity with the concept and philosophy of the Independent Living Movement.

8. Must have reliable transportation.

Job Type: Full-time

Pay: From $48,000.00 per year
