Are you looking to work as part as a Growing Hospice Team? Well Care Hospice is looking for Home Health Aides (CNA’s) to be part of our amazing team.

Essential Job Functions/Responsibilities:

• Provide total care and/or assist with personal hygiene, elimination, feeding, and mobility according to the patient’s needs and the nursing instructions as identified in the plan of care.
• Carry out the duties assigned in order to increase the patient’s level of comfort, and to maintain personal hygiene and a safe environment for the patient.
• Adheres to the written and oral plan of care for patient care as developed by the RN
• Help the patient achieve a good quality of life through:
• Encouragement of independence, eating, social contact, etc.
• Providing companionship
• Providing reassurance
• Recognize and any changes in the mental or physical status of the patient promptly to the RN Case Manager or Hospice Clinical Manager. Be aware of potential anxiety in families.
• Maintain infection control and safety practices.
• Provide support for and reinforcement of the team’s instruction for the patient’s caregivers.
• Communicate with the hospice nurse regarding services provided and significant findings regarding the patient’s functional status and change in care needs.
• Document the care provided and the patient’s response to care in accordance with policies and procedures and regulatory standards.
• Required documentation is timely, concise, and factual.
• Participate with the hospice interdisciplinary team in the development and implementation of the patient’s and family/caregiver’s plan of care.
• Participate in weekly team meetings and case conferences, as necessary to ensure appropriate services are provided to the patient.
• Maintain confidentiality of patient and family/caregiver information.
• Exhibit an attitude which promotes harmony and goodwill in the workplace.
• Promote quality, comprehensive services through a team approach.
• Participate in own professional development by maintaining required skills validation, certification, and attending educational offerings.
• Keep patient’s living area clean and orderly, as assigned.
• Adhere to the organization’s documentation and care procedures and standards of personal and professional conduct.
• Represent the agency in a professional manner in appearance and communication style, and treat all patients, families/caregivers, and other staff with respect.
• Participate in quality assessment performance improvement teams and activities.
• Respectful of patient and family/caregiver environment and patient’s personal needs.
• abuse and neglect in accordance with aide scope of practice as well as the hospice’s policy and procedures.
• Perform other duties as assigned by Manager/Director.

Position Qualifications

• Completion of certificate program in nursing care from an accredited training program with current listing as CNA-1 or CNA-2.
• Possess and maintains current CPR certification.
• 6 months experience required; hospice experience preferred but not required
• Ability to work with the team concept of patient/family care outside a hospital setting
• Ability to work independently or as part of a team.
• Must be physically capable of providing patient care in various settings to include supporting, moving, and lifting adult patients, lifting equipment, and supplies.
• Must be able to interact with patients, family members, facility staff, co-workers, and medical staff under a variety of circumstances.
• Regular, reliable attendance is essential.
• Demonstrates tact, patience and good personal hygiene.
• Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

Job Limitations

The hospice aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician’s orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, such as sterile techniques.
