What You’ll Do:
Patient Care Coordinators develop and successfully implement policies and protocols to identify, follow, and support patients in achieving the best health outcomes. Patient Care Coordinators provide and manage the critical links between our patients and supportive organizations and other medical professionals. Another key factor is understanding social determinants of health and how they impact a patient’s health status, building relationships with community organizations to help patients with social needs.
