Who You Are:

We are looking for someone who thrives in a team environment and who is passionate about serving their community. The technical requirements for this position are:

• Minimum of 1 year of professional experience in case management, or experience in similar position which works with a case management team
• Working knowledge and understanding of HEDIS and STAR quality measures and ratings
• Working knowledge of community resources in applicable geographic areas (i.e., Portland Metro area)
• Ability to work in a fast-paced environment which includes multi-tasking, prioritizing, and process-building with ambiguity
• Familiarity working with and analyzing patient and quality data (e.g., using Excel, Google Sheets, etc.)
• Ability to work within Wallace and with community organizations in collaboration and partnership
• Strong desire to work with people of diverse ethnic, social, economic, and cultural backgrounds
• Ability to work in a fast-paced environment
• Ability to sit, stand, lift, carry, walk, read fine print, and communicate in person, in writing, by telephone and electronically
• Ability to occasionally lift up to 25 lbs.
• Ability to pass a pre-employment background check specific to the position applied for

Preferred:

• Experience with assisting clients facing multiple barriers to accessing services and healthcare, including poverty, language, mental illness, or housing
• Understanding of and experience using motivational interviewing, empathic inquiry, and trauma informed care
• Experience with an electronic medical records system
• Experience with HIPAA

What You’ll Do:

Patient Care Coordinators develop and successfully implement policies and protocols to identify, follow, and support patients in achieving the best health outcomes. Patient Care Coordinators provide and manage the critical links between our patients and supportive organizations and other medical professionals. Another key factor is understanding social determinants of health and how they impact a patient’s health status, building relationships with community organizations to help patients with social needs. Duties include:

Patient Care Coordinator

• Provides outreach and coordination services such as: performing proactive outreach to identify social, behavioral, and medical needs of patients; answering patient calls
• Facilitates subspecialty and imaging appointments, including coordination of transportation as needed, processing referrals and prior authorizations
  • Prioritizes and tracks all stages of a referral requesting s and provider guidance
  • Assists patients in scheduling follow-up specialty medical and dental care appointments
  • Meets with patients to review referral procedures and requirements, addresses barriers to care and answers all patient questions
  • Cultivates relationships with community service partners, in particular the Coalition of Community Health Clinics (CCHC), and our partner safety net clinics
• Oversees and coordinates patient gap-care programs including (but not limited to) Project Access Now & Screenwise
• Assesses and identifies patients’ social determinants of health (such as housing, transportation, and access to nutritious food) to gain insight into how they may impact closing patient care gaps; works to coordinate services as needed to mitigate barriers
• Identifies high-risk patients, which includes the subpopulation of patients with multiple, chronic comorbidities, through varying methods such as collaboration with RN Case manager, PCP, patients’ caregivers, family and patients’ health plans
• Monitors patient care gaps and works 1:1 with Panel Managers & Care Teams to schedule services needed to improve and assist patients in obtaining the best health outcomes
• Provides motivational interviewing, literature, and resources as needed to support behavioral change
• Educates patients on how to use healthcare resources
• Maintains accurate and complete documentation of all care coordination services
• Timely entry of patient notes into the EMR related to services provided to the patient
• Updates policies and procedures as needed
• Maintains strict patient confidentiality
• Participates in Health Literacy events, volunteer retention and related activities
• Complies with all applicable laws and fully cooperate with the Wallace compliance program
• Participates in Continuous Quality Improvement initiatives
• Performs other duties as assigned
• The noise level in the work environment is usually moderate, similar to that of a busy office
• Duties are performed primarily in an office and clinic environment
