As a Client Care Coordinator, You Will

• Provide exceptional service to our clients and caregivers, typically 50-75 clients and 100-125 caregivers.
• Coordinate and maintain communication regarding the scheduling needs of client’s care.
• Schedule and follow up with caregivers around their availability.
• Act as the point of contact for clients, client family members, and caregivers.
• Effectively form relationships with clients and caregivers, ensuring an exceptional experience.
• Understand both the need and the skill preference of the client and appropriately match them with caregivers who possess those skills and abilities.
• Document follow up in the Company’s electronic medical records system.
• Assist and collaborate with the recruitment team with the upkeep of caregiver’s licensure and credentials.
• Collaborate with the on-call team to ensure that clients are receiving consistent care.
• Collaborate with payroll and billing team members.
• Follow up and if needed have coaching conversations with caregivers when their actions do not match the Company’s mission, vision and values or goes against the Company’s policies.
• Assist with caregiver engagement by providing support with interviews, orientation, ongoing recognition and development and educational training.
• Proactively conduct quality assurance calls with clients and caregivers.
• Follow through on assignments and commitments, support CCN goals and initiatives, and adhere to all policies and procedures.
