Trellis Supportive Care is currently recruiting exceptional Nurses to make a difference for the patients and families we have the privilege of serving.

Schedule

40 hrs/week – Full-time Monday- Friday 8:00 am – 5:00 pm with back-up oncall rotation

Salisbury, NC Regional office

Responsibilities

• Triage incoming patient and family calls from the Salisbury, NC office
• Provide direct patient homecare setting visits as necessary and as requested in the Rowan and Davie county areas
• Obtain changes for orders as needed and update the Patient’s Plan of Care and medication list according

. Manage patients’ pain and symptoms pursuant to Physician’s Orders

• Guide, support and teach patients and families care techniques over the phone and in person during visits
• Provides follow up contact with patient and/or caregiver to assess the effectiveness of telephonic intervention(s).
• Inform the patient’s physician and primary nurse of any pertinent change in condition and communicate to members of the interdisciplinary team as needed to ensure prompt delivery of quality care
• Relay requests for Patient Care Volunteer, bereavement services, chaplaincy or social work services to the appropriate Hospice clinician as needed. Refer patients/families to other services as needed
• Ensure all care provided is consistent with the patient’s plan of care

Requirements

• Excellent nursing assessment and problem solving skills
• Excellent interpersonal communication skills, both oral and written
• Exceptional customer service
• Strong clinical assessment, critical thinking, problem solving and organizational skills
• Ability to utilize independent judgment and to follow through

· Current NC license as an LPN Nurse and current CPR certification

• A minimum of two (2) years’ clinical experience preferred
• Comfort with documentation within electronic medical record, experience with Allscripts/NetSmart a plus
