Touchstone Health Services Hiring for Health Educator at Phoenix, AZ Full Time

General Summary:

The general purpose of this position is to implement prevention strategies and activities that support the program.

Essential Functions:

Implements evidence-based curriculum which includes facilitating with other staff as well as trained officials, site champions and community stakeholders. Provides a safe and inclusive environment for participants.

Develops and amends lesson plans to accommodate target populations.

Coordinates, recruits for, and facilitates youth and adult community-based groups.

Prepares necessary s, service notes and documentation to identified parties.

Researches up-to-date information on prevention topics and ensures only accurate information is disseminated .

Provides prevention resources to schools and community members.

Helps to organize and train school officials, site champions and community stakeholders on how to effectively implement prevention programs and/or build sustainability for prevention programs.

Creates materials, presentations, and trainings to promote prevention efforts, increase awareness of prevention topics, and educate the community.

Receives referrals for interventions and other behavioral, mental, physical and social needs from program participants and addresses these referrals in a timely and appropriate manner.

Potential for a $1500.oo sign-on bonus

Qualifications:

Must be at least 21 years of age

High School Diploma or general equivalency and 4 years of experience in education, youth programming, teaching, public health or behavioral health

Associate degree and 2 years of experience in education, youth programming, teaching, public health or behavioral health

Bachelors degree– any field

Must possess an active AZ Drivers License

Must possess an AZ Fingerprint Clearance Card
