Therapy Documentation Specialists Hiring for Clinical Documentation Specialist at Remote Full Time

About Us:

Therapy Documentation Specialists conducts comprehensive off-site documentation audits for companies in all 50 states who provide Physical, Occupational, and Speech therapy services. We help to ensure that their documentation is compliant with Medicare/Medicaid state and federal guidelines, as well as all private insurance companies.

The Therapy Documentation Specialist will review charts and offer recommendations to therapy providers to:

• Ensure that skilled need is justified in the therapist’s documentation
• Ensure documentation supports medical necessity
• Ensure that the therapist’s daily notes are distinctive, appropriate, and ethical
• Ensure appropriate billing compliance (CPT code and modifier selection, proper units etc.)
• Ensure therapy providers are following National Correct Coding Initiatives (NCCI)
• Ensure documentation is not missing frequency and duration of exercise prescription, physician and therapist’s signatures, dates, and total treatment time
• Ensure therapy treatments and documentation supports the CPT billing codes
• Ensure that abbreviations are not overused
• Consult during TPE reviews

Responsibilities of the Documentation Specialist include:

• Conducting patient clinical and billing chart revews (typically 2 -3 patients of various treatments-i.e. Eval, daily notes, Re-eval etc.)
• Providing a written recommendation to the therapy provider
• Providing a corrective action plan to the therapy provider using the Documentation Specialist’s audit template
• Providing resources and references to the therapy provider’s to help improve their documentation
• Providing phone support to therapy providers and/or administration regarding audit specific findings
• Providing EMR phone or video training to individuals or groups as needed
• Attend in person or video conferencing company meetings as needed

Requirements:

• Must hold a valid state license in either physical, occupational, and or speech therapy in one of the 50 US states
• Must be proficient in writing and speaking English
• Must have 10+ years of clinical experience in 3 or more clinical settings
• Must have a designed and quiet office space for phone or video conferencing
• Must be proficient in basic computer skills
• Must have a flexible schedule
• Must be ethical, responsible, and respectful to all

Job Type: Contract

Pay: $25.00 – $50.00 per hour
