What To Expect (Job Responsibilities)
- Collaborate with Case Managers and Social Workers in baseline patient assessment
- Communicate and problem solve in the development of the TOC plan
- Ensure patient and medical facility receives information on benefit coverage
- Monitor progress towards meeting the TOC goals and escalate barriers
- Coordinate necessary post discharge referrals and authorizations
What Is Required (Qualifications)
- High School Diploma
- 1 year of relevant experience in medical administrative or healthcare field
- Proficient in Microsoft Office Suite
- Excellent organizational and communication skills
- Ability to work independently and collaboratively
How To Stand Out (Preferred Qualifications)
- 0-2 years of experience in a healthcare-related service
