JOB REQUIREMENTS Education Skills and proficiency in oral and written grammar, spelling, punctuation, and basic mathematics as acquired through official completion of high school or its equivalent. Must show proficiency in medical terminology or complete course within 6 months or employment. Experience Knowledge of job content and requirements as acquired by on-the-job training and knowledge of hospital and departmental policies and procedures as acquired through orientation and preceptorship. JOB SUMMARY The Discharge Coordinator serves as the facilitator of the patient throughput/discharge process and is a valued member of the healthcare team. They collaborate with case managers and other members of the healthcare team to implement the discharge plan agreed upon by the RN, case manager, physician, patient/chose support system, and other involved members of the interdisciplinary team. The role is designed to support a wide variety of operational support functions that support discharge-planning activities.
