SUMMARY:
The Behavioral Health Patient Advocate serves as an active member of the Behavioral Health Team structure at SHHC by supporting the providers and care navigators with patient management, administrative support, and population health strategies.
The ultimate goal of the position is to improve: 1) access to care, 2) quality of care, 3) patient outcomes. She/he will work in cooperation with the Care Navigation Team to provide complete care navigation for all SHHC patients, and with the clinical practice team to ensure the highest quality patient care.
