The Hospital at Home (HAH) Hospitalist is a core member of the South Shore Health (SSH) Hospital at Home clinical team and plays a central role in admitting and caring for patients from the SSH campus (Emergency Department, and Medical / Surgical Units) to the SSH Hospital at Home program.

In their role, the HAH Hospitalist will closely collaborate with the SSH HAH Advanced Practice Clinicians, Nursing and Paramedic caregivers amongst other team members to provide direct care, patient rounding, coordination and virtual team support.

This position is a full time position with clinical duties within the Division of Hospital Medicine (DOHM) and s directly to the Associate Medical Director for Hospital at Home.

The HAH Hospitalist provides clinical consultation and support to the HAH Care Navigator RNs screening patients for admission to the SSH HAH program, facilitates admissions to the SSH HAH program, and serves as the responding clinician and caregiver for patients admitted to the SSH HAH service modeling the highest standards of care.

As the SSH HAH is a new and developing program, HAH Hospitalists will practice a majority of their initial clinical time at the brick and mortar facility with future opportunities for remote work after program launch.

It is expected that the HAH Hospitalist will at a minimum practice in the South Weymouth based command center to build relationships with SSH HAH caregivers and participate in the training of new providers.

As remote opportunities develop, the HAH Hospitalist will have HIPPA secure private space to provide care, have stable high-speed internet connection and should be available to come into the brick and mortar hospital should urgent clinical or staffing issues arise.

ESSENTIAL FUNCTIONS

Provide direct clinical care and medical decision-making to patients admitted to the HAH program (in-person or virtual, based on need;

though primary model will be virtual)

• Participates in daily HAH IDR rounds; prioritizes and participates in regular patient acquisition activities (including ED and ‘brick and mortar’ patients) in partnership with HAH Clinical Team, including clinical consultation and support to the HAH RNs screening patients for admission to the HAH program.
• Facilitates / completes history and physical examinations to the HAH program.
• Serves as a primary Attending of record and coordinator of care for patients admitted to the HAH service.
• Available to patients for urgent issues overnight, typically via virtual collaboration with a Nurse or community Paramedic.
• Oversees and / or collaborates with multidisciplinary team members according to organizational policies and practice.
• Provides oversight of HAH Advanced Practice Clinicians and Medical control for HAH Paramedics, oversight when needed for Mobile Integrated Health (MIH) Advanced Practice Clinicians and Medical control for MIH Paramedics.
• Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
• Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
• Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
• Consistently provides high quality and timely documentation, including admission and progress notes, procedure notes, and discharge summaries.
• Practices cost-effective medicine in an efficient manner, maximizing available resources.
• Discusses patient data with other physicians and professionals as appropriate in a multidisciplinary setting.
• Demonstrates expert clinical judgment skills to function in an independent role
• Takes clinical call for HAH patients as required on an alternating schedule with other HAH staff in a reasonable and equitable manner.

Virtual nighttime coverage needed occasionally, on a rotating and equitable schedule.

• Participate in clinical onboarding and training of HAH staff, new recruits, and other clinical staff new to the program.
• Provides clinical care as a member of the brick and mortar : Hospital Medicine Team when needed in accordance with expectations as outlined per Medical Staff By-laws, with all of the responsibilities / expectations as outlined in the Hospitalist position description (addendum).

JOB REQUIREMENTS

Minimum Education – Preferred

Medical Degree from an accredited institution

Minimum Work Experience

• Prior experience as hospital at home Hospitalist preferred.
• Prior experience with Hospitalist scheduling practices preferred.
• Established clinician with preferably 2 years or more inpatient attending experience.
• This position is a licensed independent practitioner credentialed through the Medical Staff Services Office.

Required Licenses

• MD / DO degree from an accredited institution
• Physician licensure, unrestricted, in the Commonwealth of Massachusetts
• Internship and Residency in Internal Medicine or Family Practice Medicine from an accredited institution

Required Certifications

• ACLS (Advanced Cardiovascular Life Support) certification as tracked by the Medical Staff Office.
• BC / BE in Internal Medicine or Family Medicine.

• Proficient in inpatient medicine, virtual care, and multidisciplinary team-based care.

• Possesses well-developed situational awareness and ability to adapt and respond quickly to urgent clinical problems. Requires personal awareness and self-management skills to manage personal stress, and to engage in personal reflection and self-correction.
• Participates in continuing education activities to maintain or enhance knowledge and skills, in particular, maintaining up to date knowledge regarding rules and regulations and CMS guidance related to the developing landscape of home hospital care.
• Works collaboratively with other major clinical departments within the Health System to ensure that patients consistently receive care that is Safe, Timely, Effective, Equitable, and Patient Centered.
• Comfort with uncertainty and holistically balancing risks of brick / mortar hospitalization and home hospitalization to provide second-to-none-quality care
• Ability manage a panel of inpatient-level acutely ill patients using combination of remote- and in-home resources.
• Must maintain a flexible schedule, including potential for nights, holidays and weekend covering pending patient care / programmatic needs.
• Must be fluent in English, both verbal and written, and experience in direct patient care is essential.
