ServerPlus Hiring for Patient Management Specialist at Remote Full Time

Full Job Description

This patient services position is a remote position where you will NOT be providing in-person services. You will help to onboard and deliver services through digital programs, and help patients manage chronic health conditions, review remote monitoring alerts, collaborate with an extended care team, coordinate care, follow and update patient care plans, and support patients in achieving their health goals. Due to the nature of contact with the client there is a confidentiality agreement that will be signed as you are dealing with client sensitive information.This position is held in high regard as you the customer service representative will be an extension of the medical staff.
You will be spending all day on a computer and/or on the phone, including making up to 30-70 outbound phone calls per day to patients

You will need to be able to start work immediately (within a few days) of accepting a position

Position is w-2.


Primary Duties and Responsibilities:
  • Primarily responsible for monitoring and managing your patient panel, providing individualized health care to assigned patients (and/or their caregivers) by phone-based communication and electronic messaging under the supervision of a licensed health care provider on a monthly basis.
  • Perform structured health assessments and follow patient care plans to support patient care, while recording all interactions electronically (entering notes, setting reminder follow-ups)

    • Create electronic alert escalations and otherwise notify physicians and nurses based on pre-established rules regarding changes in patient status.


Requirements:
  • Skilled in using computers, software, and technology

    • Happy to spend ALL DAY on the phone and in front of a computer

  • – making 30 – 70 outbound phone calls PER DAY
  • Strong verbal, phone, written and electronic communication skills.
  • Ability to work remotely and independently (i.e. outside of an office).
  • Ability to work in an environment that is free of outside noise (so others can hear you well) and private from others hearing your conversations on the phone (so your conversations are private)
  • 40 hours per week with varying shifts either Monday – Friday, 9am – 6pm or 1pm -10pm (The role does offer some flexibility to suit your needs). Willingness to make yourself available to the needs of your patients outside of typical office hours, including weekends, will be required. Monthly targets must be met and/or exceeded without fail.
  • Enthusiastic and eager to learn and grow with our organization.
  • A desire to help others.
  • Bilingual is preferred (English and Spanish).

    • Remote Patient Monitoring, Chronic Care Management & Principle Care Management experience preferred but not required.
