Serenity Care Health Group Hiring for Primary Care Nurse Practitioner Full Time

JOB SUMMARY: As an NP, you will work in collaborative practice alongside our dedicated team of health care providers and will be responsible for providing independent clinical assessment and patient care management services for our patients predominately in an acute environment. The NP will assist in direct patient care and is responsible for communication and feedback with patients, families, and referring physicians. The ideal candidate must be self-directed, function with minimal direct supervision and be familiar with working in an acute care and outpatient environment. In addition, this position will provide support in the QI/QA committee and work alongside the Chief Medical Officer to achieve clinic quality, patient satisfaction, and ensure all staff is up to date with clinic objectives such as quality boards, present progress on clinic objectives at staff meetings, etc.

ESSENTIAL DUTIES

• Conducts physical exams, assesses health status, orders and interprets tests, prescribes medications, and treats illnesses including giving injections and suturing wounds. Consults with physicians as needed and refers to physicians for more complicated medical cases or cases that are not a routine part of a NP/PA scope of work.
• Monitors therapies and provides continuity of care.
• Triages patient calls and evaluates patient problems. Responds to emergencies including use of CPR.
• Participates in QI/QA committee meetings and if needed, develops standardized procedures to ensure quality patient care and documentation.
• Documents patient information and care in medical record and may maintain department statistical database for research purposes.
• Make medical diagnoses and institute therapy or referrals of patients to the appropriate health care facilities, agencies, other resources of the community, or other physicians.
• Informs and describes treatment plans with patients, discusses diagnoses, provides counseling to patients and family members, promotes wellness and health maintenance.
• Provides continuity of care by developing and implementing patient management plans.
• Communicates with patients by telephone including answering patient questions and concerns according to protocol, discussing medication refills, further work-up, and discussing results of lab studies or x-rays.
• Achieves integrated services across the continuum of care and manages and ensures continuous improvement of all clinical practices, services, and operations by implementing systems, processes, and methods to evaluate and improve patient care
• Accountable for meeting patient satisfaction, safety, and financial goals as set by Healthcare Effectiveness Data and Information Set (HEDIS), preventive care and ‘meaningful use’ clinical goals).
• Designs and evaluates processes to improve systems and patient care results across the continuum of care in collaboration with the Chief Medical Officer

Knowledge:

• Knowledge of medical model and roles of physicians and nurse practitioners/physician assistant. Familiar with anatomy, pharmacology, pathophysiology, clinical medicine, and physical diagnosis.
• Knowledge of patient assessment techniques including taking medical histories, performing physicals, evaluating health status including state of wellness, and compliance with care recommendations.
• Knowledge of diagnosing and treating medical problems and developing care plans.
• Knowledge of documentation in medical records in confidential manner. Evaluate patient’s health status through the collection and assessment of heath data.
• Examines and treats patients.
• Order, interpret and follow-up on diagnostic studies and makes diagnosis under agreed upon protocols.
• Collects subjective and objective data. Uses data to identify health care problems and accurately documents data, actions or interventions. Develops and implement plans of care and follow-up. Documents patient’s response to plan of care.
• Provides health teaching and health counseling to patients and their families including comprehensive discussions of complex diagnostic and treatment options and end-of-life issues.
• Refers patients who need further treatment, evaluation or assistance to various medical providers and community agencies.
• Maintains clinical competence through attending educational seminars and programs and is responsible for maintaining professional certification.
• Effectively collaborates with all members of the health care team.
• Performs health assessments and preventative health measures within prescribed guidelines and physician instructions.
• Orders, interprets and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs.
• Discusses case with physician.
• Formulates and documents care plan.
• Prescribes medication or other forms of treatment.
• Participates in On-Call program which may include after-hours, weekend and holiday calls and visitations.

Qualifications:

• Active DEA license, ability to order and prescribe
• Master’s degree in Nursing
• Successful completion of accredited nurse practitioner
• RN License (For NP)
• Experience working in an acute care environment
• For NP, must be ANCC FNP
• BLS Certification
• Nursing: 4 years (Preferred)
• Bilingual in Khmer (Preferred)
