Required Qualifications:

• Minimum 1 year of experience long term care
• Bachelors level degree in following field: Art Therapy or Behavioral Sciences or Child Development or Cognitive Sciences or Community Mental Health or Counseling (MH, Vocational, Pastoral, etc.) or Counselor Education or Early Childhood Development or Educational Psychology or Gerontology or Human Development or Human Services or Marriage and Family Therapy or Music Therapy or Nursing or Pharmacy or Psychiatric Rehabilitation or Psychology or Rehabilitation Counseling or Social Work or Sociology or Special Education or Speech Therapy or Therapeutic Recreation or Vocational Rehabilitation

Preferred Qualifications:

• Experience in Case Management, Care Coordination, Discharge Planning, Social Work highly preferred
• Experience with Community Health Resources strongly preferred
• Experience initiating care plans and both subjectively and objectively conducting assessments
• Experience with educating members with chronic disease(es) strongly preferred
• Experience working in Mental/Behavioral Health strongly preferred

Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.
