Senior Doc Hiring for Care Manager at Orange, CA Full Time

The Care Manager will play a crucial role in improving the quality of life for patients with chronic conditions by coordinating and managing their long-term care. This position requires a strong background in healthcare, excellent interpersonal skills, organized and the ability to multitask.

Responsibilities:

• Monthly calls 150+ patients and fill out Care Connect forms
• Serve as the central point of contact for patients, their families, and the healthcare team
• Coordinate and facilitate communication among healthcare providers, specialists, involved in patient care
• Collaborate with behavioral health professionals to address mental health needs
• Update patients’ charts – Assist assigned providers with other tasks and coordination of care
• Monthly med reconciliation

Skills:

• Strong communication and interpersonal skills
• Excellent organizational and time management abilities
• Ability to work effectively in a team environment
• Knowledge of crisis intervention techniques
• Familiarity with behavioral health principles and practices
• Experience in hospice setting is a plus
