Under the direction of the Community Health Worker Liaison Manager,
the Community Health Worker Liaisons (CHWLs) will help build up CHW
capacity to ensure every community has access to accurate health
information to reduce the spread of disease and increase the
overall health of all San Diegans. This training program will
provide education, communication, technical assistance, resources
and outreach to local San Diego County communities. A Community
Health Worker is a trusted member of, and/or has an unusually close
understanding of the community being served. This trusted
relationship enables the CHW to serve as a
liaison/link/intermediary between health services and the
community, facilitating community access to information and
resources and improving the quality of service delivery. Utilizing
CHWs is a part of the County’s effort to ensure every community has
access to accurate health information to reduce the spread of
disease and increase the overall health of all San Diegans.

CHWLs will provide COSD-CHW Training Program to approximately 150
Community Health Workers annually. CHWLs will also provide
technical assistance and support through 5 Continuing Education
Seminars annually for CHWs. Lastly, CHWLs will help organize and
facilitate a CHW Community Coalition meeting 12 times
annually.

Position will require some community-based work and potential risk
for exposure to COVID-19. Personal Protective Equipment (PPE) will
be provided by the project.

Responsibilities
The Community Health Worker Liaisons are responsible for but
not limited to:

Program Implementation (20%)

• Build trust and rapport with Training Program participants,
  CHWs, community members, colleagues and other professionals
  representing community-based organizations and Federally Qualified
  Health Centers.
• Identify and share culturally competent COVID-19 resources for
  individuals, organizations and communities disproportionately
  impacted by COVID-19
• Engage, educate, coach, and lead Training Program participants
  in one-on-one interactions, group settings, or as needed.
• Remain informed and up to date with all new and revised
  federal, State, County and local legislation, laws, rules,
  regulations, policies, and procedures regarding COVID-19.
• Build and maintain professional networks and collaborate with
  appropriate community partners.
• Collaborate with County staff and County-employed CHWLs to
  implement any improvement needed.
• Provide support for the CHW Community Coalition to engage in
  discussions regarding the Training Program and future community
  engagement opportunities.
• Promote health equity and undertake efforts to reduce health
  disparities through engagement with community members, professional
  colleagues, and community partners.
• Assist in conducting evaluation activities and preparing
  written reports as assigned

Training Programs (20%)

• Help conduct County of San Diego approved CHW Training
  curriculum no less than five times annually either in-person and/or
  virtually
• Assist team with training implementation to include:
  • Recommend training dates & times to accommodate CHW
    schedules and needs. Training schedule shall be approved in writing
    by County of San Diego COR prior to implementation
  • Serve as trainer / facilitator where needed
  • Print handouts, informational packets, and other materials for
    events
  • Organize and transport supplies and materials
  • Field registration inquiries
  • Help with onsite event management, event set-up, and
    clean-up
  • Relay post-event feedback to team members for event improvement
    purposes
  • Identify training program graduates, County CHWLs, Contractor’s
    CHWLs, and CHW curriculum consultants from RFQ 11192 (San Diego
    County Promotores Coalition) to assist with future training program
    implementation and course instruction.
  • Distribute satisfaction surveys for event participants

Continuing Education Seminars (15%)

Community Coalition Sessions (15%)

• Help organize, coordinate and facilitate a CHW Community
  Coalition by expanding upon existing CHW coalitions, relationships
  and partnerships with groups addressing COVID-19 and providing
  other social, economic, and recovery resources.
• Help develop and disseminate agendas; and track and disseminate
  minutes to attendees.
• Track attendance

Training and Development (10%)

• Keep informed and updated on COVID-19 information and
  developments
• Attend workshops and trainings as assigned
• Provide input and feedback to improve processes, materials and
  resource needs, as appropriate

Other Duties and Responsibilities as Assigned
(20%)

• Perform other duties as assigned

Qualifications

• Bilingual (Spanish/English)- ability to read and write English
  and Spanish at level appropriate to the position to support
  maintaining accurate records and reading medical terminology
• Effective overall communication skills
• Detail-oriented with good organizational skills
• Ability to follow established protocols
• Familiarity with iPhone or smartphone and iPad or tablet
• Able to enter data into Microsoft Excel or other software
• Valid driver’s license and access to own transportation
  (mileage reimbursed) provide proof of automobile insurance and
  access to Motor Vehicle Record (DMV Pull program)
• Able to work evenings and weekends, as assigned
• Ability to maintain harmonious relations and interact
  effectively, professionally, and diplomatically with faculty,
  colleagues, to carry out assignments independently without detailed
  instructions and within established deadlines
• Ability to demonstrate a high level of cultural
  humility

Minimum Required Qualifications

• High School Diploma, GED, or equivalent
• Experience with service community
• Bilingual in English/Spanish

Preferred

• Associates or bachelor’s degree
• Previous work as Community Health Worker or Promotor(a)
• Personal experience in health education
• Understanding of public health and infectious disease
• Knowledge of San Diego County systems, organizations, programs,
  and resources
