A Patient Care Coordinator, or Care Coordinator, strives to create the best experience for patients from onboarding to discharge. Their duties include scheduling for medical appointments and other procedures, providing referrals to new care providers and educating patients about their health issues.

Patient Care Coordinator Duties and Responsibilities

• Use data to complete analytical tasks and charting tasks related to patient information
• Communicating with patients and facilities about their status and condition through emails, phone, and face-to-face conversations
• Managing patient accounts with contact information, insurance, and financial information in a confidential manner
• Scheduling patient visits and answering questions, including billing and preparation
• Ensuring services comply with state and federal regulatory requirements
• Follow up with patients and offer them our other services.
• Work with the team to provide the best patient experience.
• Be able to contact pharmacies and Interact with insurance companies.

Patient Care Coordinator Skills And Qualifications

• Two (2) years of previous experience in the same or similar role
• Medical office/laboratory background preferred
• Ability to travel to site locations as needed.
• Teamwork to coordinate with doctors and nurses and other healthcare providers
• Strong verbal communication skills to communicate with patients and the healthcare team
• People skills to connect emotionally with patients and healthcare personnel
• Experience in networking with facilities
• Time-management skils
• Strong MS Office skills

Shift: Monday-Friday

Hours: 8am-5pm

Hourly rate: $20.00-$22.00/hour
