Right at Home Loughton, Redbridge and Waltham Forest is UK’s leading Home Care provider. We are recruiting for an exciting position for a Care Coordinator. We are looking for an individual with the strength and determination to support us in taking our business to the next level.

You must have a Full UK Driving License and access to a Car.
Main Duties and Responsibilities:

• Working with Registered Manager and other team members to ensure Right at Home care operations has outstanding compliance. This is largely a field-based role to support our Clients/Caregivers in maintaining and improving our high standards of care.
• Carrying out client reviews and risk assessments
• Follow up on all concerns, referrals and liaise with other professional in a Multidisciplinary integrated care setting.
• Supervisions and monitors Caregiver’s progress i.e. observation/ spot checks
• Sign off New Caregivers making sure they are competent with their medication and Manual Handling. Shadowing / Mentoring New caregivers and supporting them making sure they are ready to work alone.
• Participate in pre-on boarding client assessments and complete care plans.
