JOB SUMMARY

The CNA will provide personal patient care services to residents under the supervision of the Home Care Supervisor within all levels of care.

• Maintains accurate and objective written and oral communications through the use of the problem-oriented record system.
• Plans and completes patient care assignments, assisting patients with all activities of daily living.
• Participates in assessing, planning, implementing and evaluating the individual patient plan of care as a part of the interdisciplinary team.
• Maintains and insures the personal rights of all patients through knowledge of and advocacy of the Patient’s Bill of Rights.
• Establishes safe and accurate working habits in the delivery of individual patient care.
• Participates in ongoing education to extend knowledge and improve skills.
• Participates in the orientation of new staff members.
• Assists in the development of Health Service programs.
• Actively participates in monitoring the medical alert and fire alarm systems.
