Full Job Description

Referrals Coordinator
ESSENTIAL FUNCTIONS
  • Responsible for collecting and collating referrals from multiple sources in an IT database.
  • Ensure complete and accurate patient registration, including but not limited to demographics, insurance information, and referral source.
  • Prioritize referrals by urgency and ensure that all referrals are addressed in a timely manner.
  • Confirm using Real-Time Eligibility that patient’s insurance is valid and active. Obtain referral, if required.
  • Make appointment reminder phone calls.
  • Communicate status of patient appointments to referring provider.
  • Return encounter notes to patients’ primary care providers, regardless of referral source.
  • Communicate with patient and clinical staff to resolve any discrepancies.
  • Point of contact for patients and health care providers for any questions or rising concerns. Assist in problem solving potential issues related to referral or initial appointment.
  • Maintain logs.
  • Follow up on missing or inaccurate information.
  • Schedule patient appointments for products or services, according to scheduling protocols.
  • Make appointment reminder calls and reschedule appointments, if needed.
  • Receive and/or relay messages to patient, clinical staff, or provider.
  • Provide support to other team members including but not limited to referrals and surgery Scheduling.
  • May assist with the completion of patient paperwork.
  • Bring to the attention of the Department Supervisor any issues that are unable to be resolved.
  • Other duties and/or tasks that may be assigned by the Department Supervisor.
REQUIREMENTS
Physical Demands

  • Sitting for long periods of time.
  • Stooping, bending, and stretching for files and supplies.
  • Occasional lifting of files or papers weighing up to 30 lbs.
  • Near visual acuity is needed to perform various clerical functions.
  • Reaching, handling, and finger dexterity which are necessary to operate various office equipment.
  • Talking and hearing are necessary for communication with patient, staff, providers, and others.
Mental Demands

  • Occasional stress from dealing with patients.
  • Determining and communicating departmental needs.
  • Occasional stress due to work demands.
  • Evaluating appropriateness of documentation.
Other Requirements

  • Work primarily at the New Albany office location and is remotely supervised.
  • Maintain the confidentiality of patient information.
  • Able to maintain professional relationships and boundaries with patients.
  • Dedication to professionalism, maturity, caring, and diplomacy.
EQUIPMENT & SOFTWARE USED
  • Computer and computer-related equipment.
  • Microsoft Suite, including but not limited to Outlook, Word, and Excel.
  • Facsimile and copy machines.
  • Telephone.
  • Other equipment and tools necessary to perform the functions of the position.
WORK ENVIRONMENT
  • Potential exposure to bloodborne, respiratory, and airborne diseases.
  • Exposure to industrial hazards as defined in Safety Data Sheets.
  • Contact with patients under a variety of circumstances, including unpredictable situations.
  • Typically, quiet with occasional periods of heightened traffic through work area, increased noise level, and interruptions.
