Lead Care Manager (Desired start date: Immediate)

Position Overview

The Lead Care Manager (LCM) is responsible for coordinating and implementing organization-wide Enhanced Care Management. In this capacity the LCM oversees and implements provision of the Enhanced Care Management (ECM) services for plan members under this LCM’s care.

The position will allow you to work with the best in your field, as well as other healthcare professionals who have worked in our industry for many years, by providing case management and advocating for their clients.

Desired Qualifications:

• High school diploma or equivalent required; bachelor’s degree in a related field preferred.
• Experience working with vulnerable populations.
• Microsoft Office product expertise required (Excel, Word, PowerPoint, Outlook)
• Bilingual
• Strong interpersonal and communication skills, with the ability to connect with diverse populations.
• Empathy, cultural sensitivity, and a passion for improving community health and well-being.
• Excellent organizational skills and attention to detail.
• Ability to work independently and collaboratively within a team.
• Valid California Driver’s license.
