Ortonville Area Health Services Hiring for RN Care Coordinator at Ortonville, MN Full Time

Ortonville Area Health Services and Northside Medical Clinic seeks an RN to join the Clinic Nursing Team to provide Clinical Care in the role of a Health Coach and Chronic Care Management. The RN functions as a Health Coach and provides nursing care across the life span in an ambulatory setting and cares for patients in all phases of preventative care, health maintenance, diagnosis, treatment and follow-up. RN provides routine nursing skills to patients as directed by their provider and the need of the clinic.

RN will work with patient and families, using education, action planning, problem solving, and motivational interviewing to support health behavioral change while bringing in the patient’s social determinants of health to find an appropriate health goal.

Qualifications:

• Demonstrates excellent communication skills with patients, family members and others on the healthcare team and engage the patient and family into the care team
• Independence, assertiveness, and critical thinking when working with patients, co-workers
• Bachelor’s Degree in nursing preferred
• Minimum of three years of professional nursing experience in a clinical care setting preferred
• Will become certified as a Clinical Health Coach and trained to support MoCA testing and Advanced Care Planning.
• The RN is required to hold a current MN and SD RN license (or eligibility is required)

This is a FULL TIME 40 hours per week position. It qualifies for our full benefits package (medical, dental, vision, PTO, retirement, etc.)
