Position Purpose: Conduct screening and risk assessment interventions per program guidelines.

• Identify special needs members through the completion of health screens and other resources.
• Work with community outreach/member advocates to coordinate member care.
• Educate providers and community resources on program components and available support services.
• Educate members with special needs to foster compliance with program and positively impact outcomes.
• Conduct site visits as appropriate for programs and provide support to other special programs.
• Develop and modify care plans in conjunction with member, member’s family and managing physician.
• Development of plan specific literature and education materials in conjunction with medical director and corporate oversight.

Education/Experience: Bachelor’s or Master’s in Social Work, Nursing, Behavioral Science, or equivalent experience. 2+ years of related care management experience

License/Certification: Valid driver’s license. RN (Registered Nurse) or licensed behavioral health clinician; LMSW or LCSW preferred

Delaware First Health Only:

Maternity Care Coordinators- Qualified candidates must reside in Kent County or New Castle County

• These positions are specific to maternity care coordination. The positions interface with pregnant members and outcomes of the program are related to improving maternal and infant health outcomes, eliminating preventable maternal and infant mortality, prevent severe maternal and infant morbidity, reduce maternal health disparities, and address member HRSN. The staff will be working to help members lower their risk for caesarean delivery, decrease the risk of postpartum complications, and improve birth spacing. The candidates should have experience in labor and delivery settings, NICU settings, Community Perinatal Care programs, or other areas of maternal health. Home visits are required.

Private Duty Nursing Care Coordinators- Qualified candidates must reside in Kent County or New Castle County.

• These positions will be working with children with medical complexity (and their families). They are charged with determining authorizations for and ensuring appropriate staffing for children who require the care of a Registered Nurse or Home Health Aide to live successfully in the community. They will work to ensure there is nursing shift coverage, appropriate durable medical equipment, and clinical appropriateness of services. Any member that has more than 8 hours per day of private duty nursing requires 2 Care Coordinators be assigned, both a Nurse and a Social Worker. This is why I am looking specifically for a Nurse for this position. Home visits are required.

Department of Correction- Qualified candidate must reside in Sussex County

• This position will work with members who are incarcerated, scheduled for release, and have been identified by the Department of Correction as high risk. It may also include members who are on supervised federal release from prison and residing at a re-entry center. The care coordinator will need to conduct visits at any of the 7 prisons in the State on a regular basis. The position will require Department of Correction clearance. There will also be the need to visit members who were recently released from prison in a community setting- home, drug treatment, MD office, after release from prison. Home visits are required. The candidate should have experience with corrections, or mental health and substance abuse populations and be willing to work with incarcerated members.

License/Certification: Valid driver’s license. RN (Registered Nurse), LMSW or LCSW required
