Compensation: $19.00 – $20.27

Schedule: Monday through Friday, 8:00 a.m. – 5:00 p.m.

Responsibilities

• Coordinate and manage the care of patients with complex medical conditions, including but not limited to coordinating appointments, medication management, and providing patient education.
• Collaborate with healthcare providers and interdisciplinary team members to develop and implement care plans that meet patient needs and goals.
• Ensure timely communication and coordination of care between healthcare providers, patients, and their families.
• Monitor patient progress, evaluate outcomes, and adjust care plans as needed.
• Maintain accurate and up-to-date patient records and ensure compliance with regulatory requirements.
• Provide patient education on disease management, medications, and lifestyle modifications.
• Assist in the development and implementation of quality improvement initiatives related to patient care.

Requirements

• Bachelor’s degree in nursing, social work, or related field required.
• Minimum of 2 years of experience in care coordination, case management, or related field.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of medical terminology, disease management, and healthcare delivery systems.
• Ability to prioritize and manage multiple tasks simultaneously.
• Strong organizational and time management skills.
• Proficiency in Microsoft Office and electronic medical records.
• Ability to maintain confidentiality and adhere to ethical standards.
