Care Coordinator Assistant – LPN – Days

Under the guidance of the Revenue Specialist and PCM Manager, the Care Coordinator Assistant is responsible for concurrent and retrospective chart, level of care and short stay reviews. The Care Coordinator Assistant also supports projects for the department in conjunction with other leaders.

Preferred Criteria

• 
  Utilization management experience
• 
  Care coordination experience
• 
  Strong organizational skills

Required Criteria

• 
  Current licensure and registration as a Licensed Practical Nurse in New York State

• 
  Five years clinical experience

• 
  Basic computer skills

• 
  Must demonstrate flexibility and good judgment

• 
  Ability to problem solve and prioritize

• 
  Ability to function as member of an interdisciplinary team
