About the Role and You
We are currently seeking a highly motivated Credentialed Diabetes Educator to join our Integrated Community Health Team on a part time basis. The ideal candidate is a dynamic, innovative and flexible Diabetes Educator, treating clients with a variety of conditions in individual or group settings.
The CDE is an active member of the multidisciplinary team providing consultation and advice pertinent to client centred care. Participation in team projects and continuous quality improvement activities are also ongoing components of the role. The service models are underpinned by encouraging clients’ self-management of their condition and within a social model of health.

The position is for 2 days a week, based at our Cranbourne/Pakenham sites.

In accordance with recent Victorian Public Health directions all employees working in the Public Health system need to be vaccinated against COVID-19 and be required to provide evidence of their vaccination status. All candidates that are recommended for appointment will be required to provide evidence they meet the vaccination criteria.
This will be by providing a copy of your immunisation history statement, which can be accessed via My Gov, or evidence that you have a vaccination appointment before 1 November 2021. This evidence will need to be uploaded to the E Recruit system.
