What is the Role

The emotional support care coordinator is a trained emotional or mental health professional that helps to manage a resident’s care, for example, the elderly or disabled. They monitor and coordinate resident’s treatment plans, educate them about their condition, connect them with emotional or mental health care providers, and evaluate their progress.

We are seeking a dedicated care coordinator to assist us with residents enrolled at the facility. You will be responsible for consulting with residents and determining their needs, developing care plans, coordinating resident-care services, educating them about their condition, and working with the care team to evaluate interventions.

The successful applicant will display a compassionate nature, be knowledgeable about emotional or mental health care practices, and provide exceptional customer service.

• Consult with residents and family members to discuss their emotional or mental health issues.
• Educate residents about their condition, medication, and give them specific instructions.
• Develop a care plan to address their personal emotional or mental health care needs.
• Consult and collaborate with other emotional or mental health care providers and specialists to set up resident appointments and treatment plans.
• Check-in on the resident regularly and evaluate and document their progress.
• Daily updating billing and attendance trackers.
• Assist the care team with developing and assessing emotional or mental health interventions.
• Attend ongoing training and courses to keep abreast of new developments in mental health care.
• Assist with securing funding for medical care as required.
• Treat residents with empathy and respect and conduct oneself in a professional manner.
• Comply with organizational guidelines health care laws and regulations.
• Complete documentation in a timely manner.

What is Required

• Experience working with clients in a Healthcare facility is strongly preferred.
• Experience working with Veterans is required
• 1 year of group therapy experience is strongly preferred
• Ability to pass a Level 2 Background Screening and Education Verification
• Ability to download and use company applications such as Microsoft Teams and BambooHR Employee on your cellphone
• Moderate Computer Literacy and ability to use Electronic Health Systems

MIHS Omega is a licensed facility through the Agency for Health Care Administrtation (AHCA) and accredited by The Joint Commission. As such, all staff requires a Level 2 background clearance as well as a clean drug screen.

Job Type: Full-time

Pay: $55,000.00 – $65,000.00 per year
