Are you an experienced Nurse Practitioner (NP) or Physician Assistant (PA) looking for a remote position in the healthcare field? If so, we have an exciting opportunity for you to join our Chronic Care Management Practice as a Remote NP/PA specializing in Medical Chart Reviews.

Responsibilities:

• Conduct thorough and comprehensive medical chart reviews for patients enrolled in our Chronic Care Management Program.
• Analyze patient medical records, history, and treatment plans to ensure compliance with established medical guidelines and standards of care.
• Identify and highlight any potential gaps in patient care, medication management, and treatment adherence.
• Collaborate with our healthcare team to provide valuable insights and recommendations for optimizing patient outcomes.
• Utilize electronic health record (EHR) systems and other relevant software to efficiently document and findings.
• Participate in periodic meetings with the healthcare team to discuss medical chart review results and implement improvement strategies.
• Maintain strict confidentiality and adhere to all HIPAA regulations during the chart review process.

Requirements:

• Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in [State].
• Proficiency in medical chart reviews and a strong understanding of medical terminology.
• Excellent analytical and critical thinking skills to evaluate patient records and identify relevant information.
• Strong attention to detail and the ability to maintain accuracy while managing a high volume of medical charts.
