Cornwall Primary Care Training Hub in partnership with Kernow Health CIC works with Health Education England and NHS England to provide at scale support and development to Primary Care.

‘Diabetes & You’ is Cornwall’s new diabetes structured self-management education programme, delivering courses to groups of people with Type 2 Diabetes. The courses will be delivered face to face or remotely.

The programme has been successfully piloted and is currently in early rollout within primary care and will continue to be developed during 2022. Full Quality Institute for Self-Management Education & Training (QISMET) approval is being sought to meet national standards for Diabetes Self-Management Structured Education.

The programme uses an underpinning philosophy of personalised care and embraces partnership working with GP practices and Primary Care networks and will be delivered by Educators who have undertaken a prescribed training programme with the Cornwall Primary Care Training Hub.

This post is a non-clinical administrative role, which is integral to the ongoing development of the programme. This role will support the team of trained educators operationally to deliver the ‘Diabetes & You’ for people with Type 2 diabetes and will be expected to complete work to a high standard and present a positive image of the organisation, whilst maintaining a high level of confidentiality.

Please note we are currently working from home, but you may be required to work at the office on occasions.

Key duties and responsibilities

• Coordinate the delivery of the programme across Cornwall, supporting central booking for core delivery plus additional PCN (Primary Care Network) activity.

• Support QISMET application to achieve accreditation for the ‘Diabetes & You’ programme.
• Support the development of the final operational model for referral/delivery process.
• Schedule all/any training courses as required.
• Book Educators to deliver programmes/courses (online, face to face, COVID permitting.
• Maintain a database of bank Educators, records of educator training, assessment, and ongoing quality standards compliance.
• Organise Educator forums, peer review and evaluation.
• Ensure patient confidentiality at all times.
• Support the delivery of the Educator training in conjunction with the Education Programme Manager, Training Hub Lead and Facilitators.
• Centrally hold and ensure Educator’s access updates curriculum resources. Review course delivery, identifying any quality issues.
• Ensure annual update of curriculum and resources via appointed expert reviewers
• Identify, engage, and support practices/PCNs to join the programme.
• Construct database and record keeping systems to ensure all related activity is recorded.
• Ensure communication and data transfer between central programme records and individual practices.
• Work with the partners and management team to achieve standards of quality, performance standards, budgets and targets without compromising levels of patient healthcare.
• Contribute towards the development and implementation of new standards, policies and procedures that are/will be required by GP practices now and in the future.
