Position Description
The Kenora Chiefs Advisory is seeking a motivated individual to support ‘high-risk’ youth (between the ages of 12 and 25) by connecting with them and supporting them to make informed decisions about their well-being in a way that is empowering, strengths based, and promotes access to Anishinaabe cultural and ceremony.
Position Title: Youth Outreach Worker
Department: Child and Youth Mental Health
Term: Full-time Permanent
Location: 240 Veterans Dr, Kenora, ON P9N 3Y5
s to: Child and Youth Services Program Manager
Summary of Duties
➢ Build professional, helping relationships with youth, and their families where appropriate, in order to support them on a path of wellness
➢ Provide individual counseling to youth and understand how to involve families in the therapeutic process where appropriate, and with consent
➢ With an understanding of the ‘Stages of Change’ intervention framework, support youth and their families around specific ways to recognize, address and resolve mental health and wellness challenges
➢ Act as liaison between service providers and at-risk/high-risk youth for the purposes of advocacy, and helping youth make positive connections with the right supports
➢ Be prepared to assist and respond to crisis situations if they arise (training provided)
Cultural Liaison
➢ Facilitate access to Anishinaabe knowledge, traditions and cultural beliefs in the healing process when appropriate
➢ Directly support youth and families to connect with Traditional Knowledge Keepers, Healers, and Elders for the purposes of accessing Ceremony, or any other cultural engagements
➢ Communicate with the Youth Wellness Hub Manager and Youth Outreach Worker Program team on all critical cases and situations
Outreach and Referrals
➢ Connect youth to services and supports via a ‘warm hand-off’ approach
➢ Provide information, education and guidance to youth, and their families with regard to mental health and addictions, healthy lifestyle choices, trauma, personal safety and human trafficking, housing, and other wellness related aspects of life
➢ Provide information, intervention options and support to parents/guardians on assisting their youth in managing mental health and wellness challenges
➢ Facilitate referrals to OTN for consults and assessments, where appropriate
➢ Participate in inter-disciplinary case conferences, RISK/risk tables, community resource meetings, and fairs
Community Engagement
➢ Network and develop partnerships with community resources both in KCA member First Nations and local urban centers such as Kenora
➢ Be responsive to concerned members of the community who may provide information about youth who are presenting at-risk/high-risk and/or appear to not be connected to supports
➢ Coordinate with Youth Mobile Crisis Workers, Crisis Workers, Youth Wellness Hub staff, and any other staff working directly with youth across service sectors to ensure that youth are receiving the support they need
Monitoring and ing
➢ Keep accurate, timely client contact records in accordance with program requirements
➢ Track significant interactions and follow-up with individual youth and their families
➢ Maintain data entry into the Outreach Record Database required by the Ministry and the program with regard to qualitative and quantitative aspects of the program
➢ Where advised, maintain data entry into KCA’s MHA data entry system, Mustimuhw cEMR (MIS) for general/daily case noting
➢ Contribute to the evaluation of service with respect to its effectiveness in meeting the community needs and achieving our mandate
➢ Additional duties as required
Qualifications:
➢ University Degree or College Diploma from a recognized post-secondary institution (Psychology, Social
Work/Social Service Worker, Child and Youth Worker), or, the equivalent combination of Lived and/or proven
Work Experience
➢ Proven experience providing both individual and group counseling to youth populations is a position requirement
➢ Experience working with, coaching, engaging, and/or supporting youth, with an emphasis on a strengths-based approach
➢ Identify as a positive role model, and committed to a lifestyle of wellness
➢ Excellent skills in the following areas: interpersonal, computer, organization, networking, presentation and communication (written and oral)
➢ Experience responding to crisis and post-crisis situations
➢ Knowledge of the Indian Residential School experience and effects of intergenerational trauma
➢ Knowledge of the Anishinaabe culture and language and experience in working in First Nations communities considered an asset
