Job Function: Responsibilities include prepping, scanning, indexing and analysis of dismissal charts, processing and scanning transcription reports, and release of information. Responsibilities also include participation in quality improvement efforts as well as monitoring and reporting of quality issues.

Job Qualifications:

· General knowledge of medical terminology

· Ability to recognize variability of handwriting

· Familiarity with forms contained in the medical record

· Familiarity with chart order and attention to detail

· Knowledge of release of information regulations and requirements

· Working knowledge of Microsoft computer applications: Word, Excel, Outlook

Job Duties and Responsibilities:

· Chart prepping, scanning and indexing into One Content (EHR)

· Chart analysis – adding signature and dictation deficiencies

· Release of information

· Process work assignments in One Content Queue’s

· Review and process transcribed reports from the transcription company

· Notify physicians of medical record deficiencies according to KHH by-laws

· Assist with quality improvement audits, as needed

· Loose sheet filing – prep, scan and analyze

· Working as part of a team to ensure completion of tasks in all areas
