North Suffolk is seeking Care Coordinators to join their team, focusing on supporting children with behavioral/emotional needs and their families to achieve independence within their communities. The Intensive Care Coordinator (ICC) role involves assessing and identifying the underlying needs of youth through regular family contact, utilizing the Wraparound Service Model to create positive impacts. ICCs help families recognize progress that leads to significant changes. For each family, the ICC serves as the primary contact for the youth’s providers and develops an individualized roadmap to guide uniform treatment among the team. This role requires collaboration with multi-disciplinary teams including PCPs, schools, DCF, outpatient therapists, and other mental health and medical providers. This position offers a unique opportunity to network, develop community connections, and gain exposure to diverse mental health, medical, and educational systems.
