Summary of Duties and Responsibilities: The Clinical Care Manager (CCM) is responsible for overseeing daily clinical operations including coordinating and directly supervising clinical staff.
Job Details and Essential Functions:

• Regularly confers with DPCS regarding clinical services, patient care needs, scheduling and reviewing documentation for accuracy and compliance with physician’s orders, regulations and organizational policies.
• At the direction of the DPCS will perform quality audits of the medical records and medical record system; verifying accurate and complete documentation.
• Assists in assuring overall quality of patient care services by coordinating aspects of patient’s care in accordance with current professional standards of practice.
• Participates with the DPCS in hiring, counseling and terminating of clinical staff
• Participates in annual supervisory field visits of nurses
• Completes annual performance evaluations for clinical staff with appropriate input from other managers, coordinators, DPCS and staff
• Responsible for coordinating and overseeing processes and people in key direct patient care roles including Intake and Triage as well as for facilitating weekly Interdisciplinary Team (IDT) meetings
• Assures monthly schedules and daily work assignments for clinical staff
• Reviews and approves timesheets for clinical staff payroll
• Responsible for coverage of triage and on-call coverage, when necessary
• Shares in administrative on-call duties
• Participates in Quality Assurance Performance Initiatives (QAPI), hospice sponsored in-services and auditing process
• Works with Intake Coordinator in scheduling admissions and admit nurse coverage
• Provides new employee clinical orientation and assures initial and ongoing staff competencies
• Carries a case load as necessary to support Case Managers
• May be assigned to any workplace setting based on HOF needs including office, RCFE, SNF, or Home
• Performs other responsibilities and duties as assigned by DPCS

Knowledge and Skills

Required

• Demonstrated competence in hospice care delivery techniques, with willingness to learn best practice and most current hospice nursing competencies and procedures
• Demonstrate a commitment to principles, practices and procedures of nursing in a hospice environment
• Perform multiple tasks in a timely manner
• Provide accurate and timely records
• Develop and maintain a professional relationship with direct s, co-workers and community partners
• Demonstrated communications and interpersonal skills, adaptability, organization, attention-to-detail, thoroughness, responsibility and leadership qualities.
• Intermediate computer skills including working knowledge of MS Office (Outlook, Word, Excel).

Desired

• Knowledge of non-cancer diagnosis criteria, State and Medicare Hospice regulations
• Public speaking, developing relationships with identified partners, groups, businesses with whom Hospice of the Foothills may have or need to establish affiliation

Education & Experience

Required

• Prior experience in a healthcare leadership role
• Minimum of 3 years of acute hospital or skilled nursing experience
• Minimum one-year Hospice, home health or community health experience within past 5 year

Desired

• Minimum of two years experience in a healthcare leadership role either as a clinical coordinator, supervisor or clinic manager
• Bachelors of Science Nursing or other applicable field of study

License / Certifications

Required

• Possess and maintain current California RN license
• Possess and maintain current CPR certificate
• Possess and maintain valid California Driver’s license, reliable automobile, and evidence of appropriate liability insurance

Desired

• Certified Hospice and Palliative Nurse certification

Job Type: Full-time

Pay: $45.33 – $57.15 per hour
