Mission Community Health Center in Mission, SD is seeking applicants for a full time Patient Care Coordinator. The Patient Care Coordinator acts as an extension of the primary care team by supporting care efforts through accessible, continuous, comprehensive, culturally effective, coordinated, compassionate care follow up. Ideal candidates must be a Registered Nurse or Licensed Practical Nurse. Location to be determined based on successful candidate’s residency. If you are a positive team player and enjoy working in a busy environment, this position is for you.
