General Description of Role and Duties

The Nursing Team at the Healthy Community Health Centers are a vital part of providing patient-centered clinical care that is accessible and affordable for all.

When you join our nursing team you will provide a safe patient care environment that supports primary care services for pediatrics through adult, dental care, behavioral health and women’s health. You will work with a team of interpreters to provide care at the highest level for those who do not speak English as their first language.

Nurses at HCHC are part of a friendly team of professionals who support our physicians, nurse practitioners, and physician assistants in achieving improved quality of care outcomes and patient volume benchmarks in a fast-paced environment.

Our team demonstrates successful professional nursing practices, patient care coordination, ethical accountability, and the preservation of patient and staff confidentiality.

The Clinical Operations Coordinator is the gatekeeper to ensuring patient care needs not associated with a direct face to face visit are met in a timely manner; facilitates walk in patient visits, and works to ensure smooth operational flow within the clinic by acting as a resource for staff and the Clinical Operations Manager.

A day at one of our HCHC sites will include collecting patient information, implementing treatment plans and evaluating the care rendered to our patients.

• Minimum of an LPN or RN degree (experience in lieu of formal degree is acceptable)
• Three years of experience in an outpatient healthcare clinic
• Supervisory experience
• Excellent communication skills
• Understanding of basic primary care services and patient centered model of care delivery
• Experience with word processing functions
• Ability to perform calmly, competently and quickly in emergency situations
• Strong customer service abilities
• Must be able to organize patient information, handle multiple tasks concurrently, possess good interpersonal skills and be able to work effectively with patients and health center staff
• Current CPR (BLS) is required.
