Health New England Hiring for Care Coordinator- New Patient Team at Springfield, MA Full Time

Contributes to the overall success of the Medicaid ACO Department ensuring specific individual goals, plans, initiatives are executed / delivered in support of the team’s business strategies and objectives. Ensures all activities conducted are in compliance with governing regulations, internal policies and procedures. This position will work as part of a team, to screen new members of a Medicaid accountable care/managed care partnership. The required screening covers demographic information, health related social needs, medical needs, disability related needs and behavioral health and substance use disorder treatment needs. Screenings will generally be conducted by phone, though in person screenings in the member’s home or other site may occasionally be required. Based on the results of the screening, the care coordinator will connect members to medical and behavioral health services.

• Serve as the contact point, advocate and informational resource for patients, care team, family/caregivers, payers and community resources.
• Documentation Responsibilities: Keeps process logs of patients triaged, dispensation of initial referral, provider scheduled, and nature of follow-up needed to best serve patient. Maintain and update documentation of contact with patient. Collect, enter, and compile data for program integration, ing, and program evaluation
• Complete Care Needs Screenings and Health Related Social Needs Screenings for all identified members
• Promote timely access to appropriate care while increasing the utilization of preventative care, managing referrals, transitions-in -care, and reducing emergency room utilization and hospitalization.
• Works independently while recognizing the importance of communicating to the clinical team and supporting team goal.
• Increase patients’ ability for self-management and shared decision -making.
• Works seamlessly with primary care and behavioral health providers, ancillary staff including other nurses, medical assistants, pharmacy, IT, front desk and any other staff to integrate patient care.
• Works closely with the primary care practice care teams in order to refer high need members to care management
• Demonstrates knowledge of applicable laws and regulations, developmental stages, pharmacology, health procedures through daily interactions with staff, physicians and patients/families.
• May require home visits

Education / Experience / Other Information (include only those that are specific to the role)

Bachelor’s degree in Health Care Administration, Public Health, Social Work, Business Administration or related field with a minimum of 2-3 years of relevant administrative and program coordination experience.

• Excellent interpersonal relational, problem solving and communication skills; both verbal and written
• Data collection and analysis skills
• Proficient computer skills including Microsoft Office Suite
• Experienced with a EMR and Care Management Systems
• Ability to work independently and as part of a team
• Considerable knowledge of principles and practices of working with diverse population
• Excellent case notes and documentation skills
• Ability to be flexible and meet performance expectations within a changing environment
• Ability to independently plan, organize and successfully execute work assignments and projects with minimal management oversight
• Ability to manage multiple priorities and manage time effectively to meet established timelines for work completion
• Valid Driver’s License
