Health Link Home Health and Hospice, a leading home health agency servicing the greater San Francisco Bay Area across multiple medical specialties and patient treatments. As an essential member of our team, you will have the opportunity to work in a collaborative and supportive environment with our clinical and office staff in San Francisco and Walnut Creek.

Currently we are seeking an experienced, self-driven Part-Time or Per Diem Licensed Home Health Aide HHA to provide care at our patient’s homes throughout Contra Costa & Alameda County and all surrounding regions. Your goal will be to improve the quality of life for your patients. All Home Health Aide services will be provided in accordance with each patient’s individual plan of care. We are looking for compassionate clinicians to join our amazing team of healthcare professionals. Our back office is dedicated to supporting our field clinicians, which truly allows them to function remotely and independently.

We are looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities

• Perform personal care (bathing, dressing, toileting, feeding, etc.)
• Assist with ambulation, transfers, range of motion
• Meeting safety needs of patients and using medical equipment safely
• Skin care to prevent breakdown of tissue
• Take vital signs

Skills

• Proven working experience as home health aide
• Ability to monitor vital signs and to collect specimens
• Familiarity with basic nutrition and personal hygiene standards
• Proficiency in English
• Nursing and health care administration skills
• Caring and compassionate personality
• Current CPR certificate
• First Aid training
• Valid driver’s licence
• High school degree

Requirements

• Valid HHA License with the State of California (will not accept HCA or CNA license)
• Valid Driver License
• Reliable Transportation
• Flexibility to cover 15-25 mile radius
• Minimum 1 year of HHA or caregiving experience
• Proof of Covid 19 Vaccination
• First Aid and CPR Certification
• High School Degree
