Opportunity:

To execute the Get Well mission to improve patient experiences and enhance care. The Get Well Virtual Lay Navigator is a non-clinical role. The lay navigator is responsible for providing peer-to-peer support, and successfully guides members/participants/patients through the health care journey. This position may support various programs such as maternity, behavioral health, primary care, health equity, COVID vaccine outreach and more.

Primary responsibilities include locating resources and services, scheduling follow-up, speciality, and wellness visits, and encouraging adherence to treatment plans. The lay navigator functions within this scope of practice and ensures services provided are consistently executed as contracted. Lay navigators work directly with members/participants/patients, in partnership with clinical teams and community organizations, under the direction of the Navigate Program Lead.

The lay navigator demonstrates a positive, dynamic, and empathetic demeanor supporting patients via text and verbally over the phone. This position must demonstrate the ability to seek out additional support when needed, problem solve in an empathetic manner, and a willingness to service and engage with a diverse group of members/participants/patients. Excellent communication and customer service skills are paramount to successfully achieving client results.

Lay navigators must complete all annual training including but not limited to privacy and HIPAA compliance training.

Responsibilities:

• Consistently execute high-quality service to patients via text message, phone calls and email communication in accordance with the required service design and volume expectations
• Serve as a guide for non-clinical inquiries and direct patients to the right resources within the hospital and in the community
• Maintain positive working relationships with patients, their families, care teams, and fellow team members
• Collaborate with the Partnership Success Client Manager & the Program Lead to optimize service offerings while supporting patients

Requirements:

• High School Diploma required; Associates Degree preferred
• Excellent time-management, problem-solving, and multi-tasking skills
• Excellent communication skills, with a strong emphasis on listening and empathy
• Ability to navigate difficult and sensitive patient conversations.
• Tech savvy. Proficiency with web-based applications and general comfort in learning new tools and technology. Tools used: Apple laptops, Navigate technology platform, Google Workspace, Zoom, PanTerra Streams, etc.
• Previous experience in support/hospitality, and/or customer service is highly preferred
• Multi-lingual: Spanish speaking preferred
• Adhere to all organizational information security policies and protect all sensitive information including but not limited to ePHI and PHI in accordance with organizational policy and Federal, State, and local regulations
