Responsibilities
Identify, facilitate, and secure access to necessary healthcare, social services benefits, and community resources.
Utilize decision support tools and guidance from supervisors to determine appropriate interventions and address healthcare and social service needs.
Communicate effectively with clients, their families, and caregivers to support care plan goals and ensure seamless care delivery.
Facilitate follow-up care after hospitalizations or emergency room visits.
Track outpatient appointments for aftercare following inpatient or acute care.
Coordinate and communicate regularly with Care Team members regarding care plan activities, referrals, transition care planning, and follow-up tracking.
Collaborate with other care team members and providers to optimize health outcomes for clients.
Liaise with payer systems and Care Coordinators to address specific patient care needs.
Conduct outreach to patients to ensure compliance with treatment plans.
Participate in team meetings to discuss outcomes improvement and adapt to the needs of the population.
Maintain accurate logs and databases related to care coordination activities.
Qualifications
Education and Experience:
High School diploma or equivalent.
1-2 years of experience in a healthcare setting.
Previous Medically-based Clerical Experience Preferred.
Familiarity with EMR/EHR systems (Cerner/PowerChart/Medent/HealthENet/HealthELink) and Microsoft Office.
Ability to collaborate effectively with a team.
Basic knowledge of medical terminology is advantageous.

Strong customer service skills, with a focus on exceeding customer expectations while upholding standards of care and confidentiality.
Ability to work independently and demonstrate self-motivation.
Comfortable with regular patient interactions.
Proficient in navigating the medical care and managed care system.
Excellent verbal and written communication skills.
Strong multitasking and organizational abilities.
This is a hybrid-remote position, and candidates must reside in WNY to be considered.
We offer a competitive benefits package including health, dental, 401K, vacation, and PTO, along with a supportive working environment.
Pay range: $18.00 to $23.00
The provided pay range represents the minimum and maximum compensation for this position. Final compensation will be determined based on various factors including market data, qualifications, and internal equity considerations.
