Here’s How You’ll Make a Difference
Streamline patient cases: You’ll handle case opening, eligibility verification, and record gathering, ensuring accuracy and efficiency.
Support patient transitions: Monitor Transition of Care (TOC) events and coordinate care plans with various stakeholders.
Contribute to accurate ing: Assist with preparing s for health plans, ensuring compliance and quality data.
Empower patients: Facilitate appointments, address benefit questions, and connect patients with necessary resources.
Promote health education: Prepare and distribute materials to educate patients about their care options and community resources.
We’re Looking For Someone Who
Brings a passion for healthcare and a commitment to excellent patient care.
Has at least 2 years of experience in a healthcare setting (specific industry experience a plus).
Navigates medical terminology with ease.
Is a whiz with Microsoft Office and patient database management.
Thinks critically and thrives in a dynamic environment.
Manages time effectively and meets deadlines consistently.
