As a key member, your primary objective is to curate a comprehensive medical history for all new patients, contributing to the seamless delivery of efficient and effective healthcare. In this position, you will engage with external electronic medical records (EMRs), ensuring that every patient’s journey begins with a well-documented foundation. Your collaboration with regional Population Health and clinical leadership will be essential in organizing and prioritizing medical records, ultimately enhancing the speed at which care teams access crucial patient information.

Core Responsibilities

Facilitate EMR Access:

Manage and maintain access to electronic medical records (EMRs) across relevant health systems in the IL market.

Patient History Compilation

Dive into EMRs for every new patient, extracting and organizing medical and social histories to enrich Oak Street’s patient records.

Efficient Documentation

Upload all discovered external information into Oak Street’s EMR, ensuring proper filing according to document types for streamlined clinical review.

Records Requests

Initiate and follow up on additional phone and fax requests for medical records in cases where direct EMR access is not feasible.

Collaborative Indexing

Collaborate with Population Health Directors, Clinical Documentation Specialists, and regional clinical leadership to index records based on clinical significance, contributing to improved ease of new patient chart reviews.
