Key Responsibilities
Collaborate with healthcare providers and staff to identify patients in need of care management, adhering to established criteria.
Conduct comprehensive assessments of patients’ holistic needs, prioritizing based on intensity and required follow-up.
Develop and implement personalized care management plans in collaboration with patients and their families, focusing on self-management and goal achievement.
Evaluate the effectiveness of care plans and revise as necessary to meet evolving patient needs and goals.
Coordinate with multidisciplinary teams to ensure seamless care transitions and address new challenges.
Utilize community resources effectively to support patient needs and promote self-management.
Facilitate patient access to community resources and provide follow-up support after hospital discharge.
Maintain accurate documentation in electronic medical records (EMR) and review utilization s to identify gaps in care.
Participate in team meetings, quality improvement initiatives, and organizational committees.
Qualifications
Experience and Skills:
Minimum of 3 years of professional medical experience, with a preference for care coordination experience.
Proficiency in electronic medical record systems.
Strong interpersonal skills and ability to work effectively with diverse populations.
Excellent organizational and multitasking abilities.
Self-motivated with the capacity to work independently.
Proficient computer skills.
Exceptional written and verbal communication skills.
Problem-solving abilities.
Spanish language proficiency preferred.
Education
Associate Degree in Medical Assisting.
Proficiency in medication indications and medical test requirements.
