In This Unique Role, You’ll

Become a trusted advocate: Collaborate with patients, families, and healthcare professionals to assess individual needs and identify appropriate community-based programs and services.

Coordinate seamless care: Make referrals, arrange service delivery, and oversee communication to ensure a smooth transition from hospital to home.

You’ll Be a Perfect Fit If You Have

A Bachelor’s Degree (any field)

2+ years of experience providing case management or similar support services

Strong knowledge of home and community-based services for older adults

Experience with discharge planning is a plus

Excellent written and verbal communication skills

The ability to work independently, manage workload efficiently, and meet deadlines

A reliable car, valid driver’s license, and a safe driving record
