Qualifications
Masters in Social Work: Required to provide comprehensive support.
Valid Driver’s License: Essential for transportation needs.
Valid Auto Insurance: Coverage for work-related travel.
AZ Level One Fingerprint Card: Required for applicants in Arizona.
COVID Vaccine: Mandatory (Exemption letter may be considered).
Flu Vaccine: Necessary during flu season.
CPR or BLS Certification: Vital for emergency response.
Physical: Medical clearance required for duties (Nevada applicants).
Valid TB Test: Within the last year (skin test) or last five years (x-ray).
Job Details
The pay rates are established within a 20-mile radius from your home, covering travel expenses. Additional compensation may be offered for patients outside your designated territory.
