Fostering meaningful connections with clients through bi-annual face-to-face or telehealth visits and addressing their healthcare needs.

Required Skills

Current LVN / LPN Licensure, demonstrating your commitment to professional standards.

Strong knowledge of Medicaid and social services to effectively navigate the complex healthcare landscape.

Excellent computer skills for efficient management and documentation of client information.

Preferred Skills

While not mandatory, the following skills and experiences would be highly valued:

Prior experience in service coordination, case management, or nursing, showcasing your understanding of client care.

Experience in customer service and case management, particularly when working with children with special healthcare needs.

Bilingual candidates are highly preferred, as it allows for better connection with a diverse range of clients.

Background in Managed Care, aiding in navigating healthcare management intricacies.

Familiarity and experience in working with Medicare or Medicaid plans, demonstrating expertise in healthcare coverage.
