As a part of our team, you will play a vital role in guiding and supporting our patients through continuous care programs, such as Remote Patient Monitoring and Chronic Care Management. Our mission is to connect with patients on a personal level and coach them towards optimal health.

Responsibilities

• Welcome and Inform:
• Warmly greet patients as they join our continuous care program(s)
• Explain the benefits and services available to them
• Educate and Empower:
• Provide clear guidance to patients on the proper use and frequency of their in-home monitoring devices
• Ensure patients have knowledge they need to manage their devices effectively
• Personalized Care:
• Collaborate with patients via phone or video visits to create personalized and comprehensive care plans tailored to their unique needs
• Overcome Barriers:
• Identify and address obstacles that may hinder a patient’s success in their healthcare journey
• Chronic Condition Support:
• Offer specialized education and coaching to help patients manage their chronic conditions effectively
• Regular Check-Ins:
• Maintain regular contact with patients to review their readings and update their care plans as needed
• First Point of Contact:
• Act as the first point of contact for non-emergent patient needs, including medication refills, appointment scheduling, and reminders
• Anticipate and Bridge:
• Anticipate patients’ needs and work to bridge any gaps in their care, ensuring a seamless healthcare experience
• Healthcare Navigation:
• Assist patients in navigating the healthcare system, including scheduling appointments, coordinating with specialists, and ensuring preventive screenings are completed
• Real-time Monitoring:
• Review and evaluate in-home device readings during normal business hours, following established protocols for identifying and addressing trends and symptoms
• Effective Communication:
• Build trust, maintain open communication, and inspire positive changes in the patient’s health
• Collaboration:
• Collaborate effectively with virtual team members and in-office staff to ensure all patient needs are met

Requirements

• Active and Unrestricted RN License in Georgia (Compact License preferred)
• Minimum of three (3) years of experience in adult health, with a preference for expertise in adult chronic health conditions (e.g., Medical Surgical, Outpatient/Clinic, Care Management)
• Proficiency in various Electronic Medical Record (EMR) systems
• Strong computer skills
• Excellent problem-solving abilities and the capacity to multitask effectively
• Outstanding communication skills, both oral and written
• Exceptional teamwork skills
• Successful completion of a clean background check and drug screening
• Comfortable with remote work while maintaining effective collaboration
