Job Description
As a Clinic Care Navigator, you will play a pivotal role in supporting patients, families, providers, and staff to facilitate seamless access to essential care. You will guide patients and their families in understanding care plans, self-management responsibilities, and encourage participation in preventive care. The primary focus is to bolster the overall efficiency of our healthcare practice.
Key Responsibilities
Utilize case management processes to ensure the delivery of high-quality care to patients, their families, and caregivers across the healthcare continuum.
Apply principles of comprehensive, community-based, patient-centered, and culturally sensitive care coordination.
Engage and coach patients, families, and caregivers in understanding, setting, and monitoring patient self-management care plans.
Document individualized care plans and care coordination in our Electronic Health Record (EHR).
Coordinate patient care by facilitating access to medical home providers, staff, and resources as needed.
Develop and maintain relationships with patients, families, and the care team to support access to the medical home.
Act as a primary contact point, advocate, and source of information for patients and community partners.
Research, find, and link patients to resources, services, and support mechanisms for their care plans and self-management needs.
Provide timely communication with patients, execute follow-up actions, and integrate information into the care plan.
Assist the care team in measuring and implementing quality improvement initiatives.
Requirements
Education:
High school diploma
CCMA/CCMA/RMA/CNA or equivalent experience in the care navigation field
Experience
2+ years in a clinical or care navigation setting
Knowledge And Skill Requirements
Self-motivated with a strong work ethic and a desire to learn
Strong time and project management skills
Ability to work effectively in a team environment
Experience with computer applications and medical terminology
Critical thinking skills and sound judgment in a fast-paced environment
Familiarity with electronic health records and registries
Empathy, mental alertness, precision, and effective communication skills
Environmental/Working Conditions
Normal office environment with talking, hearing, repetitive motions, and close visual acuity
Potential exposure to clinical/medical office, patient’s home, and/or community-based environments
Flexible work arrangements may include remote work within a HIPAA compliant and low-distraction home office.
Physical/Mental Demands
Standing, sitting, bending, stooping, twisting, and computer work
Stress involved in working with patients with multiple co-morbidities and assignments
