Key Responsibilities
Engage with patients via outbound phone calls to conduct health assessments, identify care needs, and assess eligibility for participation in care coordination programs.
Provide invaluable support to patients in overcoming care-related obstacles, including but not limited to appointment scheduling and connecting with community resources.
Collaborate with patients, network providers, referral coordinators, as well as office and hospital staff, ensuring comprehensive documentation of all activities.
Clearly communicate the available care coordination services to patients and caregivers.
Screen for physical and behavioral health needs and facilitate referrals to appropriate healthcare professionals for timely and effective intervention.
Perform a wide range of tasks to deliver meaningful data to providers, patients, and their families.
Engage in ongoing educational programs to continually enhance your skillset and maintain competency levels.
Requirements
Undertake additional job-related responsibilities as assigned.
An Associate’s degree in Healthcare or a related field; or an equivalent combination of education and experience.
A minimum of two years of experience in a customer/client/patient-focused role.
Proficiency in Microsoft Office applications and web-based technologies.
Highly Preferred Skills, Abilities, And Qualifications
Previous experience and knowledge of care coordination in the healthcare sector.
Two years of experience in a directly related position within the healthcare industry.
Familiarity with federal and state healthcare laws and requirements, particularly regarding health information protection.
One or more of the following certifications: Basic Life Support (BLS), Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Emergency Medical Technician (EMT).
Exceptional verbal, written, and interpersonal communication skills.
Proficiency in both English and Spanish, with the ability to convey complex information clearly and understand and interpret complex information from others.
Proven ability to gain acceptance and compliance from clinical staff to achieve mutually beneficial outcomes.
Strong problem-solving skills, including systematic problem analysis, drawing relevant conclusions, and developing appropriate courses of action.
Exceptional organizational and prioritization skills, with the ability to manage multiple tasks independently with minimal supervision.
