Gentiva Hiring for Nurse Preceptor at Avon, IN Full Time

We are looking for a Nurse Preceptor to join our hospice team.

We are building a pool of internal Nurse Preceptors who will travel and provide training support at our Gentiva, SouthernCare, and Heartland branches in the state of Indiana.  This is a permanent, full time position with benefits.

The ideal candidate will:

• Have 2-3 years of hospice case management experience.
• Have some experience and comfort level with in-person training, remote/video training, and ride-along training to patient visits.
• Be highly organized and flexible.
• Easily earn trust so new hires are comfortable asking questions.
• Develop training schedules and best practices.
• Demonstrate comfort and confidence in sharing expertise and opinions.
• Need to be able to do hands-on training in the field, at bedside. “Show you, now you do it.”  This is not an office position.

 This position s to the AVPO and is responsible for providing high interaction with patients as well as new hire clinicians during hospice visits.

• Facilitates the learning experience to allow preceptees to perform competently in their required responsibilities. This is achieved through mentoring, coaching, and role modeling correct behaviors.
• Performs patient home visits in accordance with physicians’ orders.
• Orients new nurses (preceptees) which includes hospice clinical policies and procedures, documentation, technology, compliance and regulatory requirements

• Current CPR certification
• Minimum 2 years’ experience in a clinical setting
• Diploma, Associate’s or Bachelor’s degree in Nursing
• Unencumbered RN license in work state assigned
